What is Blueprints of Hope?
Blueprints of Hope is an independent Colorado 501(c)(3) non-profit corporation based in Durango, Colorado, dedicated to providing community-based cancer nurse navigation, education, and supportive care programs for cancer patients,  survivors in our rural area of in Southwest Colorado. Our nurse navigation program offers one-on-one practical guidance, essential tools, and advocacy to patients, families, and caregivers who are experiencing the cancer journey.

What does Blueprints of Hope do?
We help remove or overcome obstacles, barriers to care, solve problems, locate, and provide supportive care resources that result in an improvement in the quality of life for the people we serve.

How do you do it?
We work with our clients and families along the cancer continuum to help with disease research, to locate physicians, services, supportive care, clinical trials, Oncofertility navigation, and second opinions. We educate about diagnosis, recommended treatment and care options, research, and help mobilize resources. We serve as an integrative coach within their healthcare team. We provide our clients with information and support, so they feel empowered to work with their health care providers and learn to participate in their health care system to get the care they desire.

Where do you do it?
We meet personally with clients at our office, by telephone, or e-mail correspondence. Our office hours are by appointment only.

What don’t you do?
The professionals associated with Blueprints of Hope do not provide direct medical care, make medical diagnoses, prescribe medical treatments, or make medical decisions for our clients.  We wholeheartedly endorse that the people we serve consult with their health care providers before making any medical decisions.

How will I be charged for your services
We completely understand that most individuals who have cancer are facing financial hardship. As part of our mission to assist everyone who has been touched by cancer, Blueprints of Hope offers all programming free of charge. Our services are entirely made possible by the generous donations of the communities we serve.

I live outside of Southwestern Colorado, can I obtain your services?
As time permits, we may occasionally serve individuals who live in other areas, but have a close connection to individuals who live in Southwestern Colorado. Please contact us for further information.
