By nature of living in a rural, underserved community, we recognized that supportive care programs, information, services and resources for people with cancer – if available – were fragmented. We were seeking to remove barriers to receiving timely access to healthy cancer-related information and supportive care resources when we realized that these services could be readily accessed by providing a community-based nurse navigation program to individuals who were experiencing any season of the cancer journey.
As a logical addition to its navigation services, Blueprints of Hope envisioned a simple interactive website – a connecting point – which would provide local-regional cancer information. This website would be an easily-accessible link complementing cancer and post-treatment survivorship care for individuals, families, friends, caregivers, and their providers in Southwest Colorado. We continue to bridge our resources by adding onto our supportive care programs. Please check regularly to see what’s new.
