By nature of living in a rural, underserved community, we recognized that cancer information, programs, services and resources – if available – were fragmented. Oftentimes, patients, families, caregivers and providers were unaware of the supportive care resources available to them.
Our vision was to help remove barriers to receiving timely access to cancer-related information, supportive care and resources. We realized that these services could be readily accessed by providing a community-based nurse navigation program to individuals who were experiencing any season of the cancer journey.
As a logical addition to its navigation services, Blueprints of Hope envisioned a simple interactive survivorship website – a connecting point – which would provide local-regional cancer information. This website would be an easily-accessible link complementing cancer and post-treatment survivorship care for individuals, families, friends, caregivers, survivors, and providers in Southwest Colorado.
